Project Care Coordination Centre Friesland in full swing

You may have read something about it before: at Dokterswacht we have been working on the issue “How can we organize acute (GP) care in the ANW in a smarter way” for a long time? You may have also heard of the term ‘Multidisciplinary Care’: since the summer of 2023, a home care worker has been present at Dokterswacht’s TriageCenter every weekend. This colleague immediately picks up the home care-related questions, so that the patient in question receives care faster.

Developments in the background

It was probably not visible to many, but over the past year, the GP organisations in Friesland, Groningen and Drenthe, together with chain partners, have worked very a provincial plan around the Care Coordination Centre (ZCC). This plan is about triage, organization and coordination of care and cooperation. The aim is to reduce pressure on acute care.

Through collaboration, these partners ensure that patients receive the right care quickly. Dokterswacht Friesland – as the leader of the ZCC – started implementing this plan in 2025 and is looking at how we can organize tasks within the TriageCenter (where all GP emergency questions come in in the evening, night and weekends) differently and better.

The first steps have been taken

A nice structure has now been developed to bring all information, knowledge and interests together. For example, working groups have been set up to investigate opportunities and devise plans on how we want to give concrete substance to the ZCC.

A delegation has also already gone on a working visit to Doktersdienst Groningen. They were given a look behind the scenes at the ‘TriageCenter’ of our eastern neighbors and came back to Friesland full of inspiration. The working group will now look at how these insights can help us.

In all of this, the interests of our patients and staff are paramount. We will undoubtedly work in a different way that will help us and the patient. We will of course keep you informed about how this develops in the coming period.
